Become a Patient

 

If you answer "yes" to these questions, you may qualify for free dental care.

• Are you 18 years of age or older?

• Do you live in Bucks or Montgomery County?

• Are you or your spouse / domestic partner employed (full-time, part-time, or seasonally)? Or are you a military veteran?

• Does your household earn within 250% of the Federal Poverty Guidelines? (See table to right)

 

Eligibility will be reviewed at the first visit and annually in the Spring.  Patients must continue to show proof of meeting all eligibility requirements each year to continue receiving care.

 

At your eligibility appointment, you will be required to bring with you:

• Most recent 2 months of pay stubs

• Completed Federal tax return (Form 1040) and schedules - if applicable - for the current year

• Proof of residency, such as a utility bill or signed rental lease

• Photo ID (driver's license, passport, or work ID).
